Tension pneumothorax is a life-threatening emergency in which the pleural space fills with a large collection of air. If not recognized and treated immediately, the condition can progress to respiratory insufficiency, cardiovascular collapse, and death. Tension pneumothorax is seen in the prehospital setting, emergency department, and intensive care unit.¹
Recognizing tension pneumothorax early on is crucial to survival. Treatment includes immediate needle decompression by inserting a large-bore (eg, 14- or 16-gauge) needle into the second intercostal space in the midclavicular line.² All healthcare professionals should be skilled in the rapid administration of emergency thoracic decompression,¹ however, judging needle decompression success is normally based on hearing a “gush of air” and/or vitals assessment, which is subjective and hard to hear in noisy environments.
