Emergency Medicine Physician Dr. John Croushorn shares what makes the AAJT-S from CompressionWorks a vital hemorrhage control product for the EMS industry during an EMS World Podcast.

The Abdominal Aortic and Junctional Tourniquet-Stabilized (AAJT-S) targets life-threatening hemorrhages that are in difficult-to-manage areas like the abdomen, groin, and axilla.

Dr. Croushorn emphasizes in this podcast that the AAJT-S is a game-changer in trauma care. When asked why the junctional spaces are vital to get control of, DR. Croushorn said.

“The junctional regions are challenging to treat.”

Dr. Croushorn explains that bleeding in these areas is problematic since a limb tourniquet cannot be applied for control, and achieving proximal control is difficult. Therefore, a solution is needed that can be applied quickly, is simple enough for anyone to use in under a minute, and effectively stops the bleeding. The priority is a reliable method that provides immediate hemorrhage control, allowing for reassessment afterward.

“You just take it out and you put it on. After some brief instruction, anybody can put this on in difficult circumstances in less than sixty seconds.”

Time is of the essence when dealing with an extreme hemorrhage. The AAJT-S requires no assembly. Dr. Croushorn touches on a study that the British did. “They did an hour of instruction and then in low light scenarios they were able to put it on in fifty-seven seconds.” “They put it on and it worked the right way. Now that’s the axilla, that’s over the groin, and then also the abdominal application which stops all the pelvic bleeding. Now it appears it actually stops non-compressible torso hemorrhage below the diaphragm.”

In this podcast, the use cases in both the military and civilian populations are discussed. Dr. Croushorn says the primary use case for the AAJT-S for the civilian population is pelvic fractures. In the US there are about 115,000 pelvic fractures a year. “Every medic knows how to identify pelvic fracture but no one has the technology to know who’s bleeding from that fracture until their systolic pressure starts dropping, they start showing signs of shock. The research would say that if one episode of systolic blood pressure hits 90 or below before you hit the hospital, you’ve tripled their mortality.”

The first life saved by Miami-Dade Fire Rescue using the device (the AAJT-S) was a parachutist whose chute failed to open. She fell to the ground but did not hit her head. She suffered severe injuries to her pelvis and back but remained conscious. When rescuers arrived, they applied the device to stabilize her pelvis. Noticing signs of shock, they inflated it and transported her to Jackson Hospital in Miami. There, trauma surgeons were able to save her, and she went on to make a full recovery.

The AAJT-S has the capability to make a huge difference in the pre-hospital setting.
